Which one is better- open rhinoplasty or closed rhinoplasty?

A common question that strikes in almost everyone’s mind related to rhinoplasty surgery is there any difference between open rhinoplasty and closed rhinoplasty.

These are two surgical approaches to correct the deformities of nose. Depending on the changes required to be made on patient’s nose best rhinoplasty surgeons in NYC or elsewhere either chooses open and closed rhinoplasty technique.

Closed or endonasal rhinoplasty

Closed rhinoplasty (endonasal rhinoplasty) is performed on the encircle half of the nostril lining to make changes in the underlying bone structure.

This is considered best surgical approach because changes are made within the nostril and thus not visible. Moreover, it also features a shorter recovery period as compared to open rhinoplasty.

In this approach, surgeon can easily stretch the skin and make required changes as per the need.

Open or external rhinoplasty

Open rhinoplasty also called a trans-columellar incision, in this process surgeon fold backs the nasal skin using a surgical instrument that help access the nasal structure more appropriately.

The primary benefit of open rhinoplasty is the ease of access to underlying bone and cartilage structure. This is why the accuracy result in reshaping the nose framework is more in it. Often surgeons are not able to achieve desired results with closed technique, which enable to achieve possible results.

Moreover, changes of nasal cartilage distortion and need for revision rhinoplasty for reshaping of the tip cartilages is less, if followed this approach. The approach implies intricate suturing or grafting techniques.

However, the downside of open rhinoplasty is scarring is more likely visible though it. Likewise, scars are visible but relatively small (4-5 mm) and occurs at the narrowest portion of the columella of the nose. Thus, scar is only faintly visible.

Depending upon the patient’s natural anatomy and desired results, surgeon can either choose any of the two approaches. However, many surgeons prefer closed rhinoplasty as good option because of no visibility of scars.

In many cases, however, open technique is chosen because of the improved accuracy and access benefits which overdoes the minor risk of visible scarring. In fact, most rhinoplasty surgeons regard open rhinoplasty the best choice for making changes towards difficult nasal anatomy.

So, if you are considering going for rhinoplasty to correct your nasal deformities then let your surgeon decide which surgical approach will be good for you to bring optimal results.

  2. In a primary rhinoplasty that requires a humpectomy, the dorsal aspect of the upper lateral cartilages is commonly discarded.
